Director of Health and Human Services requests approval of and authorization for the Chair to sign Amendment No. 1 to Agreement No. 210259B with Community Health Initiative Napa County, Inc., increasing the amount by $23,544 for a new maximum of $123,544, extending the term January 1, 2021, through June 30, 2022, and updating both the scope of work and compensation to make use of an augmentation to the Medi-Cal Outreach & Enrollment allocation from the California Department of Health Care Services (DHCS).

EXECUTIVE SUMMARY
The purpose of today's requested action is to approve an amendment to increase the contract maximum to make use of an augmentation to the Medi-Cal Outreach & Enrollment allocation from the California Department of Health Care Services (DHCS).
Approval of today's requested action will:
1. Increase the amount by $23,544 for a new contract maximum of $123,544;
2. Extend the term of the contract to June 30, 2022;
3. Replace Exhibit A (Scope of Work) with Exhibit A-1 to increase numbers of individuals served and increase staffing and a provision to verify addresses of individuals served;
4. Replace Exhibit B (Compensation) with Exhibit B-1 to incorporate the increased budget.
