EXECUTIVE SUMMARY
The purpose of today's requested action is to approve an amendment to increase the contract maximum to make use of an augmentation to the Medi-Cal Outreach & Enrollment allocation from the California Department of Health Care Services (DHCS).
Approval of today's requested action will:
1. Increase the amount by $23,544 for a new contract maximum of $123,544;
2. Extend the term of the contract to June 30, 2022;
3. Replace Exhibit A (Scope of Work) with Exhibit A-1 to increase numbers of individuals served and increase staffing and a provision to verify addresses of individuals served;
4. Replace Exhibit B (Compensation) with Exhibit B-1 to incorporate the increased budget.
