EXECUTIVE SUMMARY

The purpose of today’s requested action is to approve an amendment to add a new service, FirstWatch, which enables oversight over American Medical Response ambulance services. Additional Licensing Workflows are being added to the existing License Management System (LMS) to allow for ambulance permitting, automatic renewal of ambulance permitting and Emergency Medical Services (EMS) Event Reporting Workflow to increase efficiency in the current system. Additionally, the term of the original contract is modified to extend the term from July 1, 2022, through June 30, 2025, while also increasing the total contract maximum by $70,975 for a new maximum of $186,027, over a three-year term. This increase includes the upfront costs to add FirstWatch and the additional Licensing Workflows.

Future fiscal impact:

This Agreement expires on June 30, 2025. It is included in the requested Fiscal Year 2022-2023 budget and future fiscal years will be budgeted accordingly.

Consequences if not approved:

If this agreement is not approved Napa County would not be in compliance with State Health & Safety Code.  The agency will also be unable to provide accurate and timely quality improvement activities to oversee and improve the delivery of emergency medical care in the community.

ENVIRONMENTAL IMPACT

ENVIRONMENTAL DETERMINATION: The proposed action is not a project as defined by 14 California Code of Regulations 15378 (State CEQA Guidelines) and therefore CEQA is not applicable.

 

BACKGROUND AND DISCUSSION

California state law requires the use of an electronic health record system that exports data in a format that is compliant with the current versions of the California Emergency Medical Services Information System (CEMSIS) and the National Emergency Medical Services Information System (NEMSIS) standards. 
The ImageTrend data repository system meets all requirements established by CEMSIS and NEMSIS. The ImageTrend data repository system will allow the EMS Agency to meet requirements for collecting data from EMS provider agencies, including local fire departments, ground and air ambulance providers, air rescue services, and hospitals.  The data collection will help the agency meet its oversight obligations for EMS system operations and clinical quality assurance/improvement.
The costs of the ePCR data system are funded by EMS Agency oversight fees, including fees charged to the exclusive operating area (EOA) ambulance provider, hospitals, and fees associated with the Napa County Ambulance Ordinance.
This amendment will offer a new service, FirstWatch, that will greatly increase the efficiency of EMS staff related to oversight over American Medical Response.  The up-front cost of $6,000, included in the amendment is to establish a secure file transfer protocol (FTP) from the EMS Data Repository to the Online Compliance Utility, FirstWatch.  The $3,000 for the first year is for the maintenance and monitoring of the FTP between both systems. The connection to these systems is necessary for EMS Agency oversight over American Medical Response. 
Additionally, the License Management System (LMS) that is being expanded under this amendment, is directly linked to Napa County’s existing services with ImageTrend.  This allows for added efficiency for EMS personnel and provider agencies who have profiles already built into the ImageTrend system.  LMS also allows for connections directly to the National Registry of Emergency Medical Technicians, providing a more reliable way to verify candidate eligibility.  Prior to the use of LMS, applicants were required to meet in-person with EMS staff to process their certification, re-certification, or accreditation.  LMS eliminates the need for in-person appointments, reducing staff time commitments.  It also allows EMS personnel and EMS providers to submit payments online.
