EXECUTIVE SUMMARY
The California Reportable Disease Information Exchange (CalREDIE) System Data Use And Disclosure Agreement ("Agreement") sets forth the information privacy and security requirements that the Napa County Department of Public Health and the California Department of Public Health ("CDPH") are obligated to follow with respect to all CalREDIE Data collected or created within the CalREDIE System.
By renewing this Agreement, the County agrees to protect the privacy and provide for the security of all CalREDIE Data in compliance with all state and federal laws applicable to the CalREDIE Data. Permission to receive, use and disclose CalREDIE Data requires execution of this Agreement.
