EXECUTIVE SUMMARY
Innovative Health Solutions, formerly Healthy Cooking With Kids provides project coordination and implementation of a California Department of Public Health grant scope of work, convenes community partners to develop a Community Nutrition Action Plan, and provides nutritional education to low income residents and coordinate a sugar-free beverages campaign.
This Amendment increases the funding for Federal Fiscal Year (FFY) 2020-2021 by $10,000, for a new maximum of $143,352. These funds were unspent by Public Health due to the redirection of staff to COVID-19 response activities. In addition, this Amendment modifies Section 3--Specific Terms and Conditions 3.1(l) to revise the amount of Federal funding Contractor will receive under this Agreement; and replaces Exhibit B and Exhibit B-1 with B-2 (Compensation) to include a revised budget. There is no change to the amount of funding for Federal Fiscal Year 2021-2022 (October 1, 2021 through September 30, 2022), which shall remain at $133,352 per fiscal year.
