EXECUTIVE SUMMARY
Approval of the recommended action will allow the County to accept $51,144 from the Department of Health Care Services for Projects for Assistance in Transition From Homelessness ("PATH") Funding Allocation. PATH funds services to individuals suffering from serious mental illness or co-occurring serious mental illness and substance abuse disorders, and who are homeless or at imminent risk of becoming homeless.

Discretionary Justification: Health and Human Service Agency (HHSA) is not required to apply for this funding. However, acceptance of this award will allow Napa County Mental Health to continue receiving funding through the Department of Health Care Services (DHCS) for the ongoing Projects for Assistance in Transition from Homelessness (PATH) program.

Future fiscal impact: The award year ends on June 30, 2022 and is included in the requested Fiscal Year 2021-2022 budget.
Consequences if not approved: This Agreement provides for services to individuals and veterans with serious mental illness or a co-occurring mental health and substance use disorder who are homeless or at imminent risk of becoming homeless and without this funding services to this population would be reduced.

BACKGROUND AND DISCUSSION
The Napa County Health & Human Services Agency's (HHSA) Mental Health Division received its State Fiscal Year (SFY) 2021-2022 application package from the Department of Health Care Services on March 8, 2021 for its ongoing Projects for Assistance in Transition from Homelessness (PATH) Formula Grant allocation. The purpose of this allocation is to provide services to individuals and veterans who are suffering from serious mental illness (SMI) or from co-occurring SMI and substance use disorders (SUDs) and who are homeless or at imminent risk of becoming homeless.
For SFY 2021-2022, the HHSA Mental Health Division received a Funding Allocation of $51,144. The HHSA Mental Health Division will utilize this funding to:
1. Fund a portion of an Alcohol and Drug Counselor III position (0.48 FTE) in the HHSA Mental Health Division's Adult Outreach Team. This position will serve as the dedicated PATH case manager and will provide outreach; short-term case management (up to 2 months); community mental health services; screening and diagnostic services; and referrals to primary health services, job training, education services; and relevant housing services. These services will be provided at the South Napa Homeless Shelter in order to effectively help individuals access housing and other services necessary to break the cycle of homelessness caused by mental illness or co-occurring disorders; and
2. Provide housing services to PATH eligible individuals, which includes minor renovations; expansion and repair of housing; security deposits; and one-time rental payments in order to obtain housing, prevent eviction, or maintain current housing (20% of PATH funding allocation). This includes: a) PATH eligible individuals referred to the HHSA Mental Health Division's Fresh Start housing program who are awaiting income assistance; and b) PATH eligible veterans who are receiving mental health services from a HHSA Mental Health Division Forensic Mental Health Counselor while incarcerated in the Napa County jail that are transitioning back into the community and in need of housing assistance. Coordination of these housing services will occur through referral to the PATH designated case manager.
