Director of Health and Human Services Agency (HHSA) requests approval of and authorization for the Chair to sign Amendment No. 1 to Agreement No. 230171B with Mentis, Inc., for Fiscal Year 2023-2024, and each subsequent renewal, for a new contract maximum of $75,000 to provide Suicide Prevention Program services.

EXECUTIVE SUMMARY
Approval of today's action will increase the maximum contract amount by $35,000 for a new contract maximum of $75,000, and updates Exhibit A (Scope of Work) and Exhibit B (Compensation, Financial Reporting, and Budget) to incorporate new services and a new budget.
Mentis, Inc. is a local vendor.
