EXECUTIVE SUMMARY
Approval of today's action will increase the maximum contract amount by $35,000 for a new contract maximum of $75,000, and updates Exhibit A (Scope of Work) and Exhibit B (Compensation, Financial Reporting, and Budget) to incorporate new services and a new budget.
Mentis, Inc. is a local vendor.

If this agreement is not approved, Suicide Prevention Program activities will be supported to a lesser degree and MHSA funds will be under-utilized.
