BACKGROUND
California Department of Health Care Services (DHCS) released its fourth and final Providing Access and Transforming Health (PATH), Capacity and Infrastructure Transition, Expansion and Development (CITED) funding round with $145.5 million distributed to 153 organizations across 58 counties in California to further support the California Advancing and Innovating Medi-Cal (CalAIM) program development.
Grant funds are exclusively to implement Medi-Cal pre-release services under the CalAIM Justice-Involved Initiative in accordance with state law and California's approved Medicaid 1115 Reentry Demonstration Waiver. Awards were given to organizations that provide Enhanced Care Management (ECM) and Community Supports services including in-person care coordination, case management, housing support, and other vital resources for Medi-Cal members with complex needs. The funding supports capacity building, infrastructure expansion, staffing, program operations, IT needs, and improved systems for monitoring performance and data.
Following a $1.5M PATH 3 award in 2025, the Department is investing in personnel, capacity, and implementation to establish pre-release services. PATH 4 funding will help support these established services.
