Approve and authorize Amendment No. 5 to Revenue Agreement No. 200130B with California Department of Public Health (CDPH) Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), to increase the contract maximum for a new contract maximum of $2,828,653 through September 30, 2025; replace Exhibit A to reflect updated remittance address; and replace Exhibit B to reflect the increased allocation. BACKGROUND
California Department of Public Health (CDPH) Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is a federally funded nutrition program available at no cost to eligible low income pregnant, breastfeeding, and postpartum women and children up to age 5. The goal of the WIC program is to safeguard the health of low-income women, infants, and children up to age 5 who are at risk by proving nutritious foods to supplement diets, nutrition education, breastfeeding support, referrals to other health care and social services as needed while empowering families in building a healthy future through their local WIC clinic. There are nearly 2,000 WIC participants in Napa County. The monthly food benefits issued by WIC increases participants access to healthy foods and contributes to the local economy.
CDPH WIC initiated this amendment by requesting their revised exhibits with the increased allocation in years 2 and 3 be incorporated into the current Agreement.
Requested action:
Approve and authorize Amendment No. 5 to Agreement No. 200130B with California Department of Public Health, increasing the amount by $880 for a new maximum of $2,828,653; replace Exhibit A (Scope of Work) to reflect updated remittance address; and replace Exhibit B (Budget Detail and Payment Provisions) to reflect the increased allocation.

ENVIRONMENTAL DETERMINATION: The proposed action is not a project as defined by 14 California Code of Regulations 15378 (State CEQA Guidelines) and therefore CEQA is not applicable.
