BACKGROUND
The CHA presented an opportunity for Napa County Health and Human Services Agency (HHSA) to engage community members with the goal of better understanding the unique strengths and opportunities to support and improve health in Napa County. In 2023, HHSA partnered with Providence Queen of the Valley Medical Center (QVMC) to create a shared CHA; referred to as a Community Health Needs Assessment (CHNA) for hospitals. The presentation today provides as overview of the health priorities that emerged.
The NOAA is a community-based initiative that began September 2022 with the support of the Board of Supervisors, HHSA, the Napa-Solano Area Agency on Aging (AAoA), the Commission on Aging (COA), and the Napa County Healthy Aging Population Initiative (HAPI) to form a team of organizations and individuals to assess the unique needs of older adults (ages 60 and above) and identify gaps in programs and services that will enable older adults in our community to thrive. The presentation today also includes highlights of the focus areas that emerged unique to our older adult population (currently 28% of Napa County’s population).
Finally, consistent with one of HHSA’s strategic plan goals of collaborating with community partners to maximize our reach and collective impact to enhance well-being, today’s presentation will also include an overview of HHSA’s plans for bolstering the development of a Community Health Improvement Plan (CHIP) through the distillation and elevation of the CHA and NOAA, ongoing partnerships with community-based organizations, and the strategic alignment, braiding, and leveraging of funding.
