Approve and accept Revenue Agreement No. 240252B from Advocates for Human Potential (AHP) for a maximum of $3,425,528 for the term June 23, 2023 through June 30, 2027 for Behavioral Health Bridge Housing (BHBH) grant funding in order to provide treatment housing support for Behavioral Health clients. BACKGROUND
Health and Human Services Agency (HHSA) will use Behavioral Health Bridge Housing (BHBH) grant funding to provide population specific rapid re-housing (RRH) program services to individuals and families experiencing homelessness and serious mental illness (SMI), who meet criteria for the Drug Medi-Cal Organized Delivery System (DMC-ODS) due to clinical need or are enrolled in the Community Assistance, Recovery & Empowerment (CARE) Act program.
The BHBH funded programming envisions an intervention designed to help individuals and families quickly exit homelessness by providing temporary housing support that serves as a bridge to permanent, stable housing. Its core program components are bridge housing through rental assistance and case management, including referral and support services. BHBH program assistance is offered without preconditions plus the resources and services provided are tailored to the needs of the household being served.
HHSA program staff work directly with Behavioral Health clients to identify housing and to support the creation of a housing stability plan that will allow the individuals to assume full responsibility for their own housing, including full contract rent, as quickly as possible. The department will return in the future as needed with budget amendments to appropriate the use of these funds.
