Director of Health and Human Services Agency (HHSA) requests approval of and authorization for the Chair to sign Agreement No. 240149B with Archway Recovery Services, Inc. for a contract maximum of $2,295,400 for Fiscal Year 2023-2024, and a maximum of $3,625,178 for Fiscal Year 2024-2025, and each subsequent renewal, to provide substance use disorder withdrawal management and short-term residential treatment services.

EXECUTIVE SUMMARY
Approval of today’s action will allow the County to enter into an agreement with Archway Recovery Services, Inc., for the provision of substance use disorder residential treatment and withdrawal management for clients assessed, authorized, and approved for placement for substance use disorder services, as a part of the Drug Medi-Cal Organized Delivery System (DMC-ODS).

If this agreement is not approved, Napa County will be out of compliance with the Drug Medi-Cal Organized Delivery Services (DMC-ODS) waiver and will be unable to provide local substance use disorder withdrawal management and adult residential treatment services to Napa County adult Medi-Cal Beneficiaries. |

ENVIRONMENTAL IMPACT
ENVIRONMENTAL DETERMINATION: The proposed action is not a project as defined by 14 California Code of Regulations 15378 (State CEQA Guidelines) and therefore CEQA is not applicable.
BACKGROUND AND DISCUSSION
Napa County's Drug Medi-Cal Organized Delivery System (DMC-ODS) Implementation Plan requires Napa County’s HHSA to offer substance use disorder withdrawal management and short-term residential treatment services to Napa County adult Medi-Cal beneficiaries.
Archway Recovery Services in Napa County include substance use disorder withdrawal management and residential treatment services as designated by the American Society of Addiction Medicine (ASAM) assessed levels of care. Archway will provide the following levels of care:
Residential Withdrawal Management (ASAM Level 3.2-WM): Clinically managed residential withdrawal management (24-hour support for moderate withdrawal symptoms that are not manageable in outpatient setting). Withdrawal management services include group education and referral to emergency or primary medical care as needed and according to standard of care and medical protocols.
Residential (ASAM Level 3.1) Clinically Managed Low Intensity: 24-hour structure with trained and credentialed personnel providing clinically directed program activities and professionally directed treatments to stabilize and maintain substance use disorder symptoms, develop, and apply recovery skills and preparation for outpatient treatment.
Residential (ASAM Level 3.3) Clinically Managed Population Specific High Intensity: 24-hour care with trained and credentialed personnel providing clinical directed, less intense program activities and professional directed treatments to stabilize and maintain substance use disorder symptoms and to develop and apply recovery skills specific for individuals with cognitive or other functioning impairments.
Residential (ASAM Level 3.5) - Clinically Managed High-Intensity: Provides 24- hour care with trained and credentialed personnel providing clinical directed program activities and professionally directed treatments to stabilize and maintain substance use disorder symptoms, develop and apply recovery skills specific for individuals with co-occurring mental health disorders. Stabilization of multi-dimensional imminent danger and preparation for outpatient treatment.
Utilization of services will be closely monitored by a multidisciplinary team comprised of representatives from HHSA, Probation, and the contractor. Services will be provided according to local, state, and federal policies and regulations.
The approval of this agreement will help ensure Napa County adult Medi-Cal beneficiaries receive withdrawal management and residential substance abuse treatment should they be assessed for those levels of care and receive client-center, trauma-informed, high quality services.
Archway Recovery Services is not a local provider. HHSA’s Behavioral Health division issued a competitive Request for Proposals and Archway Recovery Services was selected to provide residential treatment and withdrawal management services for Napa County adult resident Medi-Cal beneficiaries.
