BACKGROUND

The Mental Health Services Act (MHSA), passed by voters in 2004, provides funding for county behavioral health systems through a 1% tax on millionaire earners. One component of MHSA is funding for Innovation (INN) Projects, designed to test new approaches to mental health services and can only be used for this purpose. In 2024, voters approved Proposition 1, amending MHSA and transitioning to the Behavioral Health Services Act (BHSA), which will begin implementation in July 2026. This Program Improvements for Valued Outpatient Treatment (PIVOT) program outlines Napa County Health & Human Services Agency - Behavioral Health (HHSA - BH) division’s plan to utilize INN funds to support BHSA implementation by mirroring an Orange County’s approved PIVOT project, creating a quicker path to Commission for Behavioral Health (CBH) previously known as the Mental Health Services Oversight and Accountability Commission (MHSOAC) approval.

This HHSA - BH PIVOT project was approved at the CBH meeting held on May 22, 2025.  The project will restructure Full-Service Partnerships by defining levels of care, establishing step-down criteria, and implementing evidence-based models such as ACT and FACT, which are requirements of the BHSA. Additionally, it will enhance behavioral health training, providing support for SmartCare EHR implementation.

If not approved, Napa County would not have access to the state-approved Innovation funding intended to support BHSA preparedness-and risks losing the allocation to reversion.

ENVIRONMENTAL DETERMINATION: The proposed action is not a project as defined by 14 California Code of Regulations 15378 (State CEQA Guidelines) and therefore CEQA is not applicable.
