BACKGROUND

In 2023, the need to complete a Community Health Assessment (CHA) presented an opportunity for Napa County Health and Human Services Agency (HHSA) to engage the community in identifying priorities to improve health in Napa County. Through that process, five priorities emerged - Behavioral Health, Access to Health Services, Economic Stability, Racial Equity and LGBTQ Inclusion, and Housing. In 2024, HHSA then engaged the community for further input to create a CHIP in which 28 strategies were identified for moving the needle on those five priority areas.

In Spring 2024, HHSA embarked upon a first ever process of identifying and aligning various funding sources to help operationalize the CHIP.  HHSA issued a series of Requests for Proposals for community-based programs/services directed at CHIP priority areas and strategies. The result was a historic award of 36 contracts with community-based organizations commencing Fiscal Year 2024-2025. Those contracts were supported by Tobacco Master Settlement Agreement (MSA) funds made available by the Board of Supervisors, and Mental Health Services Act (MHSA) funds and Opioid Settlement Funds (OSF) that HHSA made available for investments across those three funding streams totaling over $10M.

This collaborative approach has been a priority for HHSA leadership and is consistent with HHSA’s strategic plan goals of collaborating with community partners to maximize our reach and collective impact to enhance well-being and a strategic plan goal of leveraging and braiding funding.

Today’s presentation is an overview of deliverables achieved during Fiscal Year 2024-2025, which is the first year of our CHIP, that highlights the impact seen through the significant investments made by the County to programs and services delivered by community partners. The presentation will also include an overview of the anticipated funding plan and recommendations for the 5-year CHIP period.
