Director of Health and Human Services requests approval of and authorization for the Chair to:
1. Terminate current Agreement No. 170922B with Rex Adamson as of September 14, 2021 and replace it with the new Agreement; and
2. Sign a new Agreement No. 220100B with Rex Adamson, M.D. for an annual maximum of 9,000 for the term September 14, 2021 through June 30, 2022 and each automatic renewal thereof to provide medication services and conservatorship evaluations renewals for mental health clients referred by the County’s Health and Human Services Agency.

EXECUTIVE SUMMARY
The County has been under contract with Dr. Adamson for the provision of mental health services provided by Dr. Adamson at Davis Guest Home. Davis Guest Home is an advanced board and care home in Modesto, California where Napa County Mental Health places many adult clients.
The purpose of today's requested action will terminate the current Agreement with Dr. Rex Adamson and replace it with a new Agreement that includes updated terms and conditions. The new Agreement will also reflect an increase to Dr. Adamson's rates for his conservatorship work from $150/client to $200/client. There is no change to the annual maximum amount of the Agreement of $9,000 per fiscal year. Dr. Adamson is one of two physicians that Napa County is under contract with at Davis Guest Home to provide these services.

ENVIRONMENTAL DETERMINATION: The proposed action is not a project as defined by 14 California Code of Regulations 15378 (State CEQA Guidelines) and therefore CEQA is not applicable.
BACKGROUND AND DISCUSSION
Dr. Rex Adamson provides medication support services to Napa County clients residing at Davis Guest Home's Modesto facilities. Napa County Mental Health has an active contract with this facility and has many long-term clients living there. Davis Guest home provides board and care level housing with enhanced staffing and services for adult mental health clients formerly placed in long term locked settings. Davis Guest Home is designed to provide long-term stable housing for residents who are unable to maintain traditional Board and Care community placements due to both insufficient structure and support in these programs. The clients are unable to maintain alternative residential care placements because their symptoms and/or behaviors impede their ability to function at the level of independence required by these programs. The psychiatric medication support services Dr. Adamson provides to clients are an integral part of the Davis Guest home program. The facility subcontracts these services out to psychiatrists that are expected to seek reimbursement from placing counties. In addition to the medication support services provided, which may be billed to Medi-Cal, Dr. Adamson also provides conservatorship services that are not billed to Medi-Cal.
