EXECUTIVE SUMMARY
Napa County was awarded funding for a Whole Person Care (WPC) Pilot from the California Department of Health Care Services (DHCS) in FY16/17. Through the WPC Pilot, Housing & Homeless Services connected Medi-Cal enrolled individuals at risk of or experiencing homelessness to housing, medical care, case management and other supportive services designed to improve health outcomes, reduce unnecessary hospitalizations, and ensure experiencing of homelessness are brief and non-recurring. The WPC Pilot also supported the County's broader efforts to integrate disparate elements of the homeless service delivery system to ensure coordinated care, maximize efficiency, and better target and prioritize resources.

The WPC Pilot was set to expire in December 2020. Due to the 2020 COVID19 Pandemic, the Whole Person Care Pilot program was extended for one year.

In June of 2017, the County entered a contract with Resource Development Associates to conduct an external evaluation of the WPC program, per grant requirements. This contract is set to expire May 31, 2022, but more time is needed to complete the WPC evaluation because of delays due to the COVID-19 Pandemic. Approval of today's action will extend the contract to the end of this fiscal year, June 30, 2022 with no impact to the overall contract maximum.
